Qualifications

• MD/DO graduates are required.
• Board certified or eligible in Internal Medicine or Family Medicine.
• Fellowship in Geriatrics welcome but not required.
• Active, non-probationary state medical license.
• Collaborative, patient-centric orientation with leadership in care teams.
• Deliver high-quality primary care addressing medical and social needs.

Responsibilities

• Provide outpatient primary care services to seniors.
• Lead care teams with a patient-centric approach and coordinate with the care team.
• Address medical and social needs of elderly patients within the clinic.
