Our physicians focus on outpatient primary care, small panel sizes, and a supportive team to provide exceptional, patient-centered care.

Qualifications

• MD or DO graduate licensed to practice medicine.
• Active, non-probationary state medical license required.
• Fellowship in Geriatrics welcome but not required.
• Board certified/eligible in Internal Medicine or Family Medicine.
• Ability to lead care teams with a patient-centric approach.

Responsibilities

• Provide outpatient primary care to seniors.
• Lead care teams and coordinate with the care team.
• Deliver high-quality primary care and address medical and social needs.
