gobravvo seeks a Primary Care Physician (MD/DO) for an outpatient practice in The Villages, FL. This full-time role focuses on quality patient relationships and senior health in a value-based care setting.

Qualifications

• MD or DO degree required
• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Geriatric experience preferred but not required
• Experience with value-based care a plus; training provided

Responsibilities

• Provide preventive and chronic care to adults 65+, Medicare Advantage focus
• Manage daily patient panel with 12-15 patients
• Coordinate care with integrated care team including social workers and pharmacy
• Utilize EMR for documentation and care coordination
• Participate in a 3-week onboarding program with value-based care training
• Adhere to evidence-based guidelines to improve outcomes
• Contribute to a culture of collaboration and continuous improvement
