Job summary

Kintegra Health in Gastonia, NC seeks a Primary Care Provider, Physician to deliver direct and indirect care to Senior TLC participants in clinic, home, or community settings.

The role requires a medical degree with residency in Internal Medicine or Family Practice and training in geriatrics; you will perform histories and exams, order tests, manage medications, and collaborate with an interdisciplinary team; travel to nearby long‑term care facilities is expected.

Qualifications

• Medical degree and board-eligible/board-certified status.
• 3–5 years medical practice experience with at least 5 years as physician; and one year of experience with a frail or elderly population.
• Experience with frail or elderly populations preferred.

Responsibilities

• Performs health histories, physical exams, and patient teaching.
• Orders, interprets, and follows up on labs; initiates emergency measures as needed.
• Collaborates with physicians, nurses, and other healthcare professionals to support care plans.
• Visits participants in homes and hospitals as required.
• Participates in care planning and SMART goals for the participant's plan of care.
• Supports Interdisciplinary Team and promotes teamwork in all interactions.
• Attends IDT and supervisory meetings and complies with PMP documentation meetings.
• Participates in primary care on-call rotation.
