Job summary

Redwood Coast PACE is seeking a Primary Care Physician to provide comprehensive medical management for elderly participants across clinic, home, SNF, and hospital settings. You will collaborate with the IDT, NP/RN teams, specialists, and hospital staff to ensure continuity of care and high-quality outcomes.

Under the Chief Medical Officer, you will develop and oversee individualized Plans of Care, participate in quality improvement activities, and provide medical leadership and consultation to

Qualifications

• Graduate of an accredited School of Medicine.
• Board certification in internal medicine or family practice.
• CAQ in geriatric medicine preferred.
• Two years of experience with frail or elderly populations.
• California Medical Board license to practice medicine; DEA registration required.

Responsibilities

• Perform initial assessment with history and physical at intake and at least every 6 months with IDT involvement.
• Develop and monitor Plan of Care every 6 months or with changes; present to the IDT.
• Provide comprehensive primary care in clinic, homes, SNFs, hospitals, and other care settings.
• Coordinate care with hospitalists, specialists, and IDT for discharge planning.
• Lead medical oversight and consultation for the clinical team; participate in QAPI activities.
• Provide on-call medical coverage after regular clinic hours; ensure continuity of care.
