Job summary

A PACE center opening in the Stockton, CA area is seeking a Primary Care Physician to join its interdisciplinary care team.

Provide comprehensive primary medical care to frail, elderly participants with complex conditions, conduct evaluations, diagnose, and develop individualized treatment plans. Collaborate with the IDT, coordinate care across center visits, home care, and referrals; ensure accurate EMR documentation. Competitive compensation and strong benefits accompany this role.

Qualifications

• MD or DO required.
• Board certification in Internal Medicine, Family Medicine, or Geriatric Medicine preferred.
• Active California medical license or ability to obtain prior to start.
• Current DEA registration.
• Experience with geriatric populations or PACE programs preferred.

Responsibilities

• Provide comprehensive primary medical care to a panel of PACE participants.
• Conduct evaluations, diagnose and manage acute and chronic conditions, and create individualized treatment plans.
• Serve as an active member of the interdisciplinary care team (IDT), collaborating with nursing, social work, rehabilitation therapy, dietary, recreation, and personal care staff to plan and update each participant's plan of care.
• Participate in regular IDT meetings.
• Coordinate participant care across settings, including center-based visits, home care coordination, and referrals to specialists or hospital care.
• Order and interpret diagnostic testing, prescribe and manage medications, and refer to specialty care as needed.
• Provide clinical oversight and collaborative support to Nurse Practitioners and other clinical staff.
• Maintain accurate, timely documentation in the electronic medical record (EMR).
• Support quality improvement initiatives and CMS/PACE program compliance.
• Participate in an on-call rotation as needed.

Requirements

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
• Board certification in Internal Medicine, Family Medicine, or Geriatric Medicine preferred.
• Active, unrestricted California medical license, or ability to obtain prior to start.
• Current DEA registration.
• Experience with geriatric or medically complex populations preferred.Prior experience in a PACE program or similar interdisciplinary care model preferred.
