Job summary

CHS Recruiting is seeking a Physician for Geriatric Primary Care in the Cleveland, Ohio area. The role focuses on delivering high-quality primary care services to elderly patients living in long-term care facilities and private homes.

You will manage a defined panel, conduct health assessments, develop care plans, coordinate with nursing staff, and participate in a flexible 4- or 5-day schedule with no evenings or weekends.

Qualifications

• Ohio medical license required.
• DEA registration required.
• Board Certified or Eligible in Internal/Family/Geriatric Medicine.

Responsibilities

• Serve as primary care provider for geriatric patients in long-term care and home settings.
• See patients in long-term care facilities and private homes within assigned territories.
• Develop personalized care plans and health management strategies.
• Conduct health assessments and adjust treatment plans as needed.
• Provide education to patients and families on health management and self-care.
• Coordinate with advanced practice providers and nursing staff; ensure efficient scheduling.
