Job summary

CHS Recruiting is seeking a Physician - Geriatric Primary Care in Youngstown, OH to provide comprehensive primary care to geriatric patients in long-term care facilities and private homes. The role includes care planning, health assessments, and collaborative work with a multidisciplinary team.

Typical schedule is 4- or 5-day, Monday to Friday with no evenings or weekends, and minimal on-call. Compensation includes a base salary of $250,000–$275,000 plus bonuses and comprehensive benefits.

Qualifications

• Ohio medical license is required.
• DEA license required.
• Board Certification in Internal/Family/Geriatric Medicine or eligible.

Responsibilities

• Provide primary care to geriatric patients in long-term care facilities and private homes.
• Conduct health assessments and develop personalized care plans.
• Evaluate patient conditions and adjust treatment plans as needed.
• Perform minor procedures and wound care as required.
• Educate patients and families on health management and self-care practices.
• Coordinate with nurse practitioners, nurses and other healthcare professionals.
