CHS Recruiting is seeking a Physician focused on geriatric primary care in the Cleveland metro area. The role involves serving as the primary care provider for a geriatric panel located in long- term care facilities and patients' homes, with flexible geographic assignment and optional inclusion of house calls.

Responsibilities include health assessments, creating care plans, evaluating treatment responses, and coordinating with NPs and other clinicians.

Qualifications

• Medical licensure required or eligible in Ohio.
• Board certification in Internal Medicine, Family Medicine, or Geriatric Medicine preferred.

• $180,000 to $225,000+ Base Salary, depending on experience

COMPANY PROFILE:

This organization is dedicated to delivering convenient, high-quality primary care to geriatric patients in the comfort of wherever they call home. They currently boast a team of more than 50 physicians and nurse practitioners who ensure that patients receive the best medical care possible without the challenges of traveling to a provider’s office. Their services include, among others, primary care, chronic care, psychiatric care, post-acute care, and transitional care.

POSITION DESCRIPTION:

The physician serves as the primary care provider for a dedicated panel of geriatric patients.

Patients are located in long-term care facilities (ALFs / ILFs) and private homes. Physicians are provided a convenient geographic area in which all patients are located and there is flexibility in regard to whether house call patients are included in the panel.

Physicians see a variable number of patients per day, dependent upon type of visit, travel time, and patient needs. Dedicated schedulers put together daily schedules / routes for the providers to ensure efficiency and appropriate time for every visit.

Specific duties of the role include, but are not limited to:

• conduct health assessments to identify patient needs
• develop personalized care plans
• regularly evaluate patient conditions and responses to treatment
• adjust treatment plans as necessary
• perform minor procedures and wound care as required (and as settings permit)
• educate patients and families on health management, disease prevention, etc.
• provide guidance on lifestyle modifications and self-care practices
• coordinate with nurse practitioners, nurses, and other healthcare professionals

The practice uses a mobile-based EHR with AI functionality to reduce administrative time.

On-call is phone triage only with very low volume and providers rotate the on-call responsibility, typically one (1) night per month.
