Job summary

LIFE at Kinder Park, part of Trinity Health PACE, is seeking a Primary Care Physician (PCP) to provide comprehensive medical care to geriatric participants in a nursing home setting. You will collaborate with an interdisciplinary team to assess, plan, implement, and adjust care plans, ensuring high-quality, patient-centered care.

You will coordinate referrals to specialists, oversee hospital admissions when needed, and maintain detailed documentation while participating in ongoing professional

Qualifications

  • MD or DO with active license to practice in PA.
  • BLS certification required; CDS and DEA registration required.
  • Strong communication and collaboration with interdisciplinary teams.
  • Ability to exercise independent judgment and adapt to change.
  • Willingness to travel within the Central PA region as needed.

Responsibilities

  • Provide initial and ongoing primary medical care to PACE participants in the nursing home setting.

Position Purpose

Under the supervision of the medical director, the Primary Care physician (PCP) provides primary medical care to PACE Organization (PO) participants, including assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct participant medical care, and evaluation of effectiveness of the plan of care. The PCP also functions in a collegial relationship with the other health care professionals, making independent decisions regarding medical needs and health regimens and carries out medical procedures as indicated.

What You Will Do
  • Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments; attend morning updates and report changes in participants’ baseline status to appropriate staff on a daily basis.
  • Provide initial and ongoing participant assessments, care plan development, and implementation of care plan. Integrate the primary care treatment plan into the overall plan of care developed by the interdisciplinary team.
  • Refer participants to medical specialist as indicated. Manages all care of participants in the nursing home: Provides regular visits to manage the care of participants in the nursing home in collaboration with the Interdisciplinary Team; maintains communication with nursing home staff for continuous monitoring of PACE participants on a case‑by‑case basis; admits nursing home participants to hospital when necessary.
  • Oversees and manages participant’s use of medical specialist and inpatient care. Communicates and follows up with specialist regarding outcome of visit.
  • Function as a member of the interdisciplinary team. Maintains regular attendance at, and participants in interdisciplinary team meetings; communicates participant changes, collaborates on care planning decisions and coordination for 24‑hour care delivery.
  • At the direction of the Medical Director reviews medical outcomes/statistical and utilization data, including reviewing of necessity of hospitalizations, and use of specialists. When problems are identified he/she works with the Medical Director to establish best practices and works with clinical staff and interdisciplinary team to make appropriate procedural and or operational changes to avoid unnecessary hospitalizations and improve the health of participants.
  • Complete documentation and uses appropriate diagnostic coding.
  • Advise the Medical Director in ways and means to establish better accountability of PO services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands.
  • Assist with the development of policies and procedure, standards of care; performs on‑going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
Minimum Qualifications
  • Doctorate Degree (Includes PhDs): MD or DO. Current and active plenary physician license to practice in state of PO required. CDS Certification and DEA Registration required. Current BLS required.
  • Board Certification Internal Medicine or Family Practice required. Board Certification in Geriatrics preferred.
  • A minimum of one (1) year working with the frail or elderly population required.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.
  • Must be action‑oriented, have business acumen, manage conflict well, be customer focused, have high decision quality, flexibility to adapt to ongoing change and have organizational agility. Ability to work with minimal supervision and exercise independent judgment.
  • Possess interpersonal skills to drive collaboration, commitment and productivity when working with cross functional teams, customers and end users. Must be comfortable functioning in a virtual, collaborative shared leadership environment.
  • Demonstrates superior written and verbal communication and presentation skills appropriate for audience comprehension. Able to communicate effectively with individuals and groups representing diverse perspectives.
  • Comprehensive to expert proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc. Ability to use other software as required to perform the essential functions of the job.
  • Excellent organizational skills. Ability to perform multiple duties and functions related to daily operations and maintain excellent customer service skills. Ability to perform frequent detailed tasks and provide immediate service with frequent interruptions. Ability to change and be flexible with work priorities. Strong problem‑solving skills.
  • Must exhibit critical thinking skills and possess the ability to prioritize workload.
  • Position requires periodic travel within the Centers geographic region
