• At least 5 years of post-residency clinical experience.

Responsibilities

• Provide comprehensive outpatient primary care to patients 65 and older.
• Manage chronic conditions and develop personalized treatment plans.
• Perform preventative care, wellness visits, and routine screenings.

• Maintain accurate and timely electronic health records documentation.
