Job summary

Community Care is seeking a Physician to join a mission-driven team in Racine, Wisconsin. This full-time role supports geriatric and primary care, collaborating with an interdisciplinary team and supervising advanced practice providers.

Duties include exams, care plan development, referrals, and health education. The position requires DEA registration, NPI, WI driver’s license, and board certification in Internal Medicine or Family Practice (geriatrics preferred).

Qualifications

• Licensed physician with DEA registration.
• NPI required.
• Board certification in Internal Medicine or Family Practice; geriatrics preferred.
• Minimum 1 year recent primary care experience with at-risk or elderly population.

Responsibilities

• Functions as a member of the interdisciplinary team and participates in the development of the comprehensive plan of care.
• Supervises nurse practitioner or clinical nurse specialist at designated site.
• Provides preventive, episodic, and chronic care; arranges referrals; performs examinations and health status evaluations.
• Orders tests, interprets results, and follows up; maintains documentation of care services.
• Educates members and families regarding health maintenance and illnesses.
• Travel locally as needed using personal vehicle.
