Job summary

Community Care is seeking a full-time Physician for the day shift based out of our Waukesha office. The role focuses on geriatric and primary care within a collaborative interdisciplinary team, supporting adults with complex health needs in the community.

Responsibilities include preventive care, acute and chronic illness management, and coordinating care plans with nurses and specialists. A Doctorate in Medicine is required, with DEA, NPI, CPR/AED, and Wisconsin driver’s license.

Qualifications

Doctorate degree in Medicine required.
DEA registration required.
NPI and CPR & AED required.
WI Driver’s License required.
Board Certification in Internal Medicine or Family Practice; advanced certification in Geriatrics preferred.
Minimum of 1 year of recent primary care experience with at-risk or elderly population.

Responsibilities

Be a supervising physician with the nurse practitioner or clinical nurse specialist at designated site.
Provide preventive and episodic health care, health promotion, and referrals as indicated.
Conduct physical, functional, and mental status evaluations; manage acute and chronic illnesses.
Order and interpret tests, adjust medications, and document health care services.
Educate patients and families on health maintenance and disease management.
