Job summary

Medix seeks a Nurse Practitioner to join an outpatient primary care team focused on serving the senior population within a value-based care model. The NP will provide primary care in one clinic, collaborating with physicians and an interdisciplinary team to deliver preventive care and improve long-term health outcomes.

Qualifications

• Active Family Nurse Practitioner (FNP) license required.
• Current BLS or ACLS certification.
• Minimum of 2 years of Nurse Practitioner experience.
• Background in Family Medicine, Internal Medicine, Primary Care, or Geriatrics.
• Experience with geriatric patients is preferred.
• Value-based care or Medicare Advantage experience is a plus.
• Experience with eClinicalWorks EMR is a plus (training provided).

Responsibilities

• Provide comprehensive outpatient primary care to adults 65+.
• Diagnose, treat, and prescribe medications for acute and chronic conditions.
• Collaborate with physicians, care managers, and clinical staff to coordinate patient care.
• Perform comprehensive assessments and develop individualized care plans.
• Focus on preventative care and chronic disease management.
• Complete patient documentation and follow-up using the EMR.
• Conduct telehealth visits or administrative work when schedules allow.
