TRU Community Care is seeking a Nurse Practitioner or Physician Assistant to provide primary care for geriatric participants in the PACE program in Lafayette, CO. The role involves clinic and home visits, collaboration with an interdisciplinary team, and a rotation on after-hours call.

The ideal candidate has 2+ years of primary care experience, Colorado NP/PA license, DEA registration, and Medicare/Medicaid credentials, with the ability to bill for palliative care and symptom management.

Qualifications

• NP or PA with 2+ years of primary care experience.
• Current Colorado NP/PA license and valid DEA registration.
• Medicare/Medicaid provider credentials required or eligible.
• Ability to bill insurance for palliative and symptom management visits.
• Experience with frail/elderly patients preferred.

Responsibilities

• Provides diagnosis, treatment, and ongoing management for PACE participants and hospice patients.
• Delivers primary care in clinic and at participants' homes when needed.
• Responds to urgent clinical needs during regular hours and participates in after-hours call rotation.
• Collaborates with physicians, nurses, social workers, and therapists for coordinated care.
• Provides palliative care and symptom management as appropriate.
• Maintains safety and adheres to health/safety protocols.
