Job summary

Vital Healthcare Solutions is seeking a flexible, compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide behavioral health medication management to residents in skilled nursing facilities and other residential settings.

This floating role travels to multiple facilities, with assignments typically requiring travel up to one hour from home and no weekends. It offers a collaborative team environment focused on compassionate, quality patient care and a full benefits package.

Qualifications

• Active PMHNP certification and applicable state licensure.
• Experience in psychiatry or behavioral health, with preference for geriatric/long-term care.
• Excellent clinical judgment, communication and interpersonal skills.
• Willingness to travel up to one hour from home to facilities.
• Flexible, dependable and able to adapt to changing assignments.
• Genuine passion for improving behavioral health and quality of life in older adults.

Responsibilities

• Provide psychiatric evaluations, medication management and ongoing behavioral health care to residents in long-term care facilities.
• Assess, diagnose and treat mental health conditions in geriatric populations.
• Develop and adjust individualized treatment plans based on assessments and patient response.
• Monitor medication effectiveness and potential side effects, and outcomes.
• Collaborate with facility staff, physicians, and families to support quality patient care.
• Maintain accurate clinical documentation and comply with regulatory standards.
• Travel to assigned facilities as needed and adapt to changing schedules and coverage.
