Job summary

Responsibilities include patient assessments, managing acute and chronic conditions, coordination of care, and ensuring smooth transitions from hospital to post-acute settings.

Qualifications

• MD or DO from an accredited institution with residency in Internal Medicine or Family Medicine.
• Board certification or board eligibility in primary care or geriatric discipline; post-acute care experience is beneficial.
• Massachusetts medical license and DEA registration; credentialing for Salem Hospital and affiliated facilities.
• Experience managing complex conditions in older adults in SNF/long-term care settings.
• Strong clinical judgment and familiarity with quality metrics and regulatory standards.

Responsibilities

• Conduct patient assessments and manage acute and chronic conditions.
• Coordinate treatment plans and ensure continuity of care from hospital to post-acute settings.
• Collaborate with nursing staff, rehabilitation teams, case managers, and families to optimize outcomes.
• Document in the medical record (Epic) and participate in care conferences.
• Engage in quality improvement initiatives and adhere to regulatory standards.
• Maintain reliable on-site presence and participate in educational activities.
