Reliant Medical Group in Worcester County seeks a Physician for our Skilled Nursing Facility, offering part-time or full-time options. The role blends on-site SNF care with remote administrative tasks, collaborating with an advanced practice clinician to manage goals of care and track patient progress.

You will provide value-based care for long-term and rehab patients, coordinate with a multidisciplinary team, and discuss care goals with families.

Qualifications

• National Board Certification in Internal Medicine or Family Practice required.
• Massachusetts medical license required.
• Graduate of an Accredited Medical School required.
• Maintain BLS certification.
• Geriatrics Fellowship desirable.
• Hospice and Palliative Care Fellowship desirable.

Responsibilities

• Provide value-based care to Skilled Nursing Facility patients.
• Hybrid work model combining on-site and remote tasks when appropriate.
• Lead multidisciplinary rehab team including APN/physician, nursing, therapy, and social work.
• Discuss goals of care with patients and families and caregivers.
• Monitor and manage acute and chronic conditions common in nursing homes.

Education
