Job summary

North East Medical Services in San Jose, CA is seeking a Physical Therapy Assistant to support PACE Center therapists in delivering treatment and maintaining participant mobility for older adults.

The role requires PTA program completion and CA Board licensure/certification, a valid driver’s license for home visits, and current BLS/First Aid. Strong EMR proficiency (Epic) is preferred.

Qualifications

• PTA graduate from CA Board approved program.
• Current licensure/certification as required by CA Board.
• Valid driver’s license for home/community visits.
• BLS and First Aid certification current.

Responsibilities

• Assist or supervise therapeutic exercises and procedures per PT direction.
• Deliver direct PT treatments including gait training and ADL support.
• Prepare and maintain a safe rehab gym and equipment.
• Document progress and communicate with Rehab Team Lead.
