Job summary

North East Medical Services is seeking a qualified Physical Therapy Assistant to support PACE Physical Therapists in treatment and maintenance of participant mobility and pain relief. You will assist exercises, deliver modalities, and maintain the rehab gym while ensuring client safety and data accuracy.

Fluency in English is required; BLS and First Aid are necessary. Experience with older adult populations and home health visits are preferred, with Epic proficiency used for documentation and

Qualifications

• Licensure or Certification as required by State guidelines and California Board of Physical Therapy.
• Current BLS and First Aid required.
• Minimum one year of experience working with frail/older adult population.
• At least one year of clinical experience in Physical Therapy in a health care setting.

Responsibilities

• Assist or supervise exercises and therapeutic procedures as requested by the therapist.
• Deliver physical modalities (PAMS) as instructed by the therapist.
• Set up equipment and maintain a clean rehabilitation space.
• Observe participants during treatment and report progress to the PACE Center Manager and Rehabilitation Team Lead.
• Instruct participants on home exercise programs, gait training, transfers and use of assistive devices.
• Provide home visits and home health as needed.
• Use Epic effectively as a workflow expert and resolve issues.
