Job summary

Qualifications

• Completed an accredited PTA program.
• State licensure as required by Michigan.
• Experience in geriatric post-acute care preferred.

Responsibilities

• Provide PTA services per the Physical Therapist plan of care.
• Document care to support medical records and reimbursement.
• Coordinate with other disciplines for total patient care.
• Ensure delivery of quality care and participate in discharge planning.
• Maintain clinical expertise through continuing education.
• Assist in care conferences and staff communications.
