Job summary

Chatham Hills Subacute Care Center is seeking an Experienced Physical Therapist to join our facility and deliver patient-centered rehabilitation services. You will assess patients, design and manage treatment plans, and document outcomes in collaboration with an interdisciplinary team.

The role emphasizes geriatric care experience, excellent documentation, and leadership over PT Assistants while maintaining confidentiality and professional standards. Weekend and weekday shifts are available.

Qualifications

• Must have graduated from an accredited Physical Therapy college or university.
• Must hold a valid professional license and registration for the state of practice.
• Experience with long-term care and/or geriatric care preferred.
• Proficient in documentation writing and patient progress tracking.
• Collaborates with all disciplines to provide patient-centered care.
• Supervises Physical Therapy Assistants and adheres to confidentiality laws.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct PT care.
• Establish and administer treatment programs aligned to patient goals.
• Document patient condition and progress, including evaluations and discharge notes.
• Educate patients and families on physical therapy methods and safety.
• Collaborate with interdisciplinary team and supervise PT Assistants.
