Job summary

Oasis Therapy Services in Burr Ridge, IL seeks a Physical Therapist to provide rehabilitation services in a skilled nursing facility. This PRN position offers flexibility to help residents improve mobility, safety, and independence. You will collaborate with an interdisciplinary team to deliver high-quality care.

Qualifications include an active IL PT license, accreditation from a PT program, current CPR, and strong communication and documentation skills. Geriatric or SNF experience is preferred.

Qualifications

• Active Illinois Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, skilled nursing, or rehabilitation experience is preferred.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide treatment focused on strength, balance, gait, transfers, mobility, pain management, and fall prevention.
• Monitor residents' progress and modify treatment plans as needed.
• Complete discharge planning and provide recommendations for continued care.
• Educate residents, families, and caregivers on exercises, safety, and assistive equipment.
• Supervise Physical Therapist Assistants when assigned.
• Complete accurate Net Health documentation and collaborate with the interdisciplinary care team.
