Job summary

AltaMed Health Services in Los Angeles is seeking a Physical Therapist for the PACE program to deliver high-quality care that restores function, improves mobility, relieves pain, and supports independence for frail elderly and disabled patients.

You will evaluate mobility, develop individualized plans of care, and collaborate with a diverse team. California PT license and BLS are required; bilingual English/Spanish/Cantonese/Mandarin is preferred.

Qualifications

• Must hold California Physical Therapist license.
• Minimum of 1 year experience with frail elderly and disabled populations.
• Valid BLS certification per AHA/Red Cross guidelines.

Responsibilities

• Evaluate mobility dysfunction and develop a plan of care tailored to the patient.
• Restore function, improve mobility and independence, and prevent or limit permanent disability.
