AcuteCare Health System LLC's BoldAge PACE program is hiring a Physical Therapist to deliver high-quality PT services to participants in home and center settings. You will assess, plan, and implement therapy within the interdisciplinary team to promote independence and safe mobility.

Qualifications

• Advanced degree in Physical Therapy from an accredited school.
• Current license to practice Physical Therapy in the state.
• Bilingual and fluent in Hmong preferred.
• Two years of experience on an interdisciplinary team in healthcare is preferable.
• 1 year of experience working with a frail or elderly population preferred.

Responsibilities

• Provides high quality care as a member of the IDT.
• Performs in-person assessments at enrollment and ongoing as determined by the IDT.
• Assesses need for adaptive equipment and home safety modifications.
• Educates participants, caregivers, or family to maintain community-based support.
• Attends interdisciplinary team meetings and documents care plans.
