Job summary

Laguna Hills Health and Rehabilitation is seeking a dedicated Physical Therapist to evaluate, plan, and implement individualized therapy programs for residents in a skilled nursing facility. The goal is to maximize function and improve quality of life through collaborative care with physicians, nurses, and occupational therapists.

Qualifications

• Doctorate or master's degree in physical therapy from an accredited program.
• Prior experience in a skilled nursing facility or similar setting preferred.
• New graduates with strong clinical skills are encouraged to apply.

Responsibilities

• Conduct thorough evaluations of residents’ physical abilities and limitations.
• Develop individualized treatment plans based on assessment findings.
• Implement physical therapy interventions to improve strength, mobility, balance, and coordination.
• Monitor and document residents' progress and adjust treatment plans as necessary.
• Work closely with physicians, nurses, and occupational therapists.
• Communicate effectively with residents and their families regarding treatment plans and progress.
