Job summary

Bluestonetherapy is seeking a Physical Therapist (PT) for Sioux Falls, SD, to provide rehabilitation services to adult and geriatric patients at Dow Rummel. This role focuses on evaluating and treating patients within skilled nursing environments, ensuring development of effective care plans.

The ideal candidate will possess a current PT license and demonstrate strong clinical reasoning and communication skills, with experience in skilled nursing or post-acute rehabilitation being preferred.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled physical therapy treatment focused on functional and measurable outcomes.
• Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning.
• Supervise Physical Therapy Assistants in accordance with licensure and regulatory standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.
