Job summary

Bluestonetherapy is seeking a licensed Physical Therapist (PT) in Sioux City, IA to provide rehabilitation services for adult and geriatric patients at The Heritage of Northern Hills. This role emphasizes evaluating and treating patients in a skilled nursing and rehabilitation setting.

You will perform evaluations, develop care plans, and oversee Physical Therapy Assistants while ensuring positive patient outcomes through collaborative efforts.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long-term care, or post-acute rehabilitation preferred.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled physical therapy treatment focused on functional and measurable outcomes.
