Job summary

Blue Stone Therapy is seeking a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at United Living Community in Brookings, SD. The role focuses on evaluating and treating functional mobility, gait, balance, and pain management in a skilled nursing and post-acute setting.

You will develop and implement care plans, supervise therapy assistants where applicable, and collaborate with a multidisciplinary team to support positive patient outcomes across partner

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license (or eligibility for licensure) in applicable state.
• Experience in skilled nursing, long‑term care, or post‑acute rehabilitation preferred.

Responsibilities

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide skilled physical therapy treatment focused on functional and measurable outcomes.
• Address gait training, balance, mobility, strength, range of motion, pain, and discharge planning.
• Supervise Physical Therapy Assistants in accordance with licensure and regulatory standards.
• Maintain appropriate patient schedules per established guidelines.
• Complete timely, accurate documentation to support quality care and reimbursement requirements.
• Collaborate with interdisciplinary team members and facility staff to support positive patient outcomes.

Physical Therapist (PT) - Brookings, SD

Blue Stone Therapy is seeking a Physical Therapist (PT) to provide rehabilitation services to adult and geriatric patients at United Living Community in Brookings, SD.

This role focuses on evaluating and treating adult and geriatric patients in a skilled nursing and post‑acute rehabilitation setting. The Physical Therapist is responsible for patient assessments, care plan development, and implementation of treatment programs designed to improve gait, balance, mobility, strength, range of motion, pain management, and overall functional independence.

This position will serve multiple partner communities, including Bethel Lutheran in Madison, with travel between locations as needed.
