Job summary

The Chatham Hills Subacute Care Center in New Jersey is seeking an Experienced Physical Therapist to join our rehabilitation team. The PT will assess referred patients, develop and implement treatment plans, and document progress with a focus on patient-centered care.

The ideal candidate has graduated from an accredited PT program, holds state licensure, and is capable of supervising PT assistants while collaborating with interdisciplinary teams to deliver high-quality geriatric rehabilitation.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must hold a valid state license and registration.
• Experience supervising Physical Therapy Assistants will be required.
• Experience in long-term care or geriatric settings preferred.

Responsibilities

• Assess referred patients and determine rehabilitation needs.
• Develop and administer individualized treatment plans with goals.
• Document patient progress and educate patients and families.
• Collaborate with all disciplines to provide patient-centered care.
• Supervise and guide PT Assistants.
