Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking an experienced Physical Therapist (PT) to assess and treat patients requiring rehabilitation services. You will establish and monitor treatment plans, document progress, and educate patients and families within a collaborative care team.

The role includes supervising PT assistants, ensuring regulatory compliance, and delivering high-quality patient-centered care.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, recertification and discharge documentation.
• Collaborates with all disciplines to provide patient-centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time and adhere to scheduled hours.
• Compliance with confidentiality, HIPAA and Federal Residents Rights.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer a treatment program with goals based on patient capacity.
• Set realistic goals, document patient status, and educate patients and families.
• Oversee and supervise PT Assistants.
• Collaborate with disciplines to ensure best patient-centered care and outcomes.
