Job summary

The Chatham Hills Subacute Care Center in New Jersey seeks an experienced Physical Therapist to assess referred patients and provide direct PT care within a multidisciplinary team. You will establish treatment plans with clear goals, document patient progress, and educate patients and families on therapy options.

The role requires an accredited PT program graduate with a valid license, experience in geriatric or long-term care, and strong documentation skills.

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must hold a valid PT license and registration for state of practice.
• Experience in long-term or geriatric care preferred.
• Strong documentation and notes proficiency.
• Collaborates with all disciplines for patient-centered care.
• Punctual, professional, and compliant with confidentiality.
• Experience with Case Mix Index or PPS scheduling is preferred.

Responsibilities

• Assess referred patients and provide direct PT care.
• Establish and administer treatment programs with goals.
• Document patient condition and progress.
• Educate patients and families on therapy methods.
• Oversee and supervise Physical Therapy Assistants.
• Collaborate with other disciplines for integrated care.
