Job summary

Chatham Hills Subacute Care Center in Hillside, NJ is seeking an Experienced Physical Therapist to provide rehabilitation services in our facility. The PT will assess patients, develop and implement treatment plans, document progress, and educate patients and families.

The ideal candidate has graduated from an accredited PT program, holds a NJ license, and has experience with long-term/geriatric care. You will supervise Physical Therapy Assistants and collaborate with all disciplines to deliver

Qualifications

• Must have graduated from an accredited Physical Therapy program.
• Must have a valid NJ license and registration.
• Experience supervising PT assistants.
• Experience in long-term/geriatric care preferred.
• Strong documentation skills.
• Experience with Case Mix Index and PPS scheduling is a plus.

Responsibilities

• Assess referred patients and provide direct patient care.
• Develop and administer individualized treatment plans.
• Set goals according to patient capacity and tolerance.
• Document patient progress and discharge planning.
• Educate patients and families about physical therapy.
• Supervise and collaborate with Physical Therapy Assistants.
• Maintain confidentiality and HIPAA compliance.
