Job summary

Chatham Hills Subacute Care Center in New Jersey is seeking an experienced Physical Therapist for our facility.

You will assess referred patients, establish and administer individualized treatment plans, set attainable goals, document progress, and educate patients and families in appropriate therapy methods. The role also involves supervising Physical Therapy Assistants and collaborating with the care team to ensure optimal outcomes.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Valid PT license and registration in the state of practice.
• Experience supervising PT assistants is preferred.
• Experience with long-term or geriatric care is preferred.
• Strong documentation skills and patient record maintenance.
• Collaborates with all disciplines to provide patient-centered care.

Responsibilities

• Assess referred patients requiring rehabilitation services.
• Provide direct patient care and develop treatment plans.
• Set goals and document patient condition and progress.
• Educate patients and families in physical therapy methods.
• Supervise and coordinate with Physical Therapy Assistants.
• Complete evaluations, weekly progress notes, and discharge documentation.
