Job summary

Ave Maria Home in Bartlett, TN is seeking a licensed Physical Therapist to provide comprehensive PT services for a geriatric population. You will conduct assessments, develop care plans, and implement interventions within an interdisciplinary team.

You will document, educate families, and collaborate with the IDT to optimize functional independence while maintaining Ave Maria Home’s standards and mission.

Qualifications

• Bachelor's degree in physical therapy and active state licensure.
• Experience working with geriatric populations preferred.
• Ability to assess patient needs and develop a comprehensive plan of care.
• Competency in using electronic medical records.
• Strong communication with patients, families and IDT members.

Responsibilities

• Perform initial evaluation and develop treatment plan.
• Document all clinical activities per guidelines.
• Deliver PT interventions and adjust as needed.
• Monitor patient progress and adjust care plans.
• Educate patients, families and caregivers.
• Supervise PTA, PT/PTA students and rehab tech.
• Participate in quality management and survey activities.
• Coordinate with IDT and attend care conferences.
• Update IDT and physicians on patient status.
• Maintain licenses and professional memberships.
