Job summary

Ave Maria is seeking a dedicated Physical Therapist to provide comprehensive PT services in an interdisciplinary setting, including assessment, treatment planning, and interventions aligned with state and federal guidelines and Ave Maria's values.

The role emphasizes program development, quality improvement, patient and family education, supervision of PTAs and students, and ongoing professional development within a collaborative care environment.

Qualifications

• Bachelor’s degree in Physical Therapy and current PT license.
• Experience with geriatric population preferred.
• Ability to assess patient needs and develop comprehensive care plans.
• Competency with electronic medical records and documentation.
• Strong communication with patients, families, and IDT team.

Responsibilities

• Complete patient screenings and initial evaluations.
• Document clinical notes, recertifications, and discharge summaries.
• Provide patient and family education to support functional independence.
• Supervise PTA, PT/PTA students, and rehab tech per guidelines.
• Participate in quality management and IDT activities.
• Coordinate with IDT and update team on patient status.
• Pursue continuing education and maintain licenses/memberships.
