Job summary

Water’s Edge Rehab and Nursing Center at Port Jefferson is seeking an Experienced Physical Therapist (PT) to join our facility team. The PT will assess referred patients, develop treatment plans, and supervise assistants while coordinating with the physician and care team.

The role emphasizes documentation, patient-centered care, and collaboration with all disciplines. Prior LTC/geriatric experience is preferred, and confidentiality and professional standards are required.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Must have a valid professional license and registration for state of practice.
• Oversee and supervise Physical Therapy Assistants.
• Experience with long-term care and/or geriatric care preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on capacity and tolerance.
• Document patient condition and educate patients and families in therapy methods.
• Perform evaluations, progress notes, recertification and discharge documentation.
