Job summary

Sayville Rehabilitation & Nursing Center is seeking an experienced Physical Therapist to join Luxor at our facility in New York. You will assess referred patients, develop and manage treatment programs, and document progress while educating patients and families.

In this role you will supervise Physical Therapy Assistants, collaborate with interdisciplinary teams, and ensure clinically excellent, confidential care for residents.

Qualifications

• Graduate from an accredited Physical Therapy program.
• Valid state license/registration to practice.
• Oversee and supervise Physical Therapy Assistants.
• Geriatric/long-term care experience preferred.
• Strong documentation and record-keeping skills.
• Perform evaluations, progress notes, recertifications and discharge documentation.
• Collaborate with all disciplines for patient-centered care.
• Demonstrate exceptional patient service and clinical excellence.
• Punctual and professional with scheduled hours.
• Perform duties assigned by the Director of Rehabilitation.
• Maintain confidentiality and HIPAA compliance.
• Experience with Case Mix Index and PPS scheduling preferred.

Responsibilities

• Assess referred patients requiring rehabilitation and provide direct patient care.
• Establish and administer treatment programs with goals based on capacity.
• Document patient condition and progress; educate patients and families.
• Supervise Physical Therapy Assistants and coordinate with team.
• Collaborate with all disciplines to deliver best patient-centered care.
• Maintain professional conduct, documentation standards and confidentiality.
• Attend to additional duties as directed by supervisor.
