Pathways PACE in Greeley, Colorado is hiring a Physical Therapist for a brand-new location. This full-time role offers predictable weekday hours (Mon-Fri, 8:00 AM-5:00 PM) and a starting salary of $90,000-$100,000 with a $5,000 sign-on bonus.

Qualifications

• Graduate of an accredited entry level PT program
• Current CPR certification
• Valid Colorado driver's license and proof of insurance
• One year working with elderly population, home care or geriatric experience

Responsibilities

• Evaluate functional abilities and develop individualized treatment plans
• Recommend home modifications and adaptive equipment to improve safety and independence
• Collaborate daily with physicians, nurses, therapists and social workers
• Help participants maintain meaningful daily activities and quality of life
• Launch and help establish best practices for a new PACE program
