Job summary

Pathways PACE in Greeley, Colorado, seeks a Physical Therapist to help shape the future of care for older adults in Northern Colorado. This is a founding clinical role within a brand-new location and offers weekday hours plus a collaborative team environment.

Qualifications

  • Graduate of an accredited entry level program in Physical Therapy
  • Current CPR certification

Responsibilities

  • Evaluate functional abilities and develop individualized treatment plans
  • Recommend home modifications and adaptive equipment to improve safety and independence
  • Collaborate daily with physicians, nurses, physical therapists, social workers, and other disciplines
  • Help participants maintain meaningful daily activities and improve quality of life
  • Play a role in launching a new PACE program and establishing best practices

Salary: $80,308 - $87,900 annually

Qualifications

We're looking for someone who is passionate about helping older adults thrive.

Required:

  • Graduate of an accredited entry level program in Physical Therapy
  • Current CPR certification
  • Valid Colorado driver's license and proof of insurance
  • One year working with elderly population, home care or geriatric experience

Preferred:

  • Ability to work effectively with culturally, economically, and educationally diverse populations
  • Experience working with interdisciplinary teams
  • An interest in preventative, community-based care
