Pathways PACE in Greeley, Colorado, is hiring an Occupational Therapist for a brand-new location. This full-time role runs Monday–Friday, 8:00 AM–5:00 PM, with a salary of $90,000–$100,000 annually and a $5,000 sign-on bonus.

Responsibilities

• Evaluate functional abilities and develop individualized treatment plans.
• Collaborate daily with physicians, nurses, physical therapists, social workers, and other disciplines.
• Help participants maintain meaningful daily activities and improve their quality of life.
• Play a key role in launching a new PACE program and establishing best practices.

Qualifications

We're looking for someone who is passionate about helping older adults thrive.

Required:

• Graduate of an accredited Occupational Therapy program
• Current CPR certification
• Valid Colorado driver's license and proof of insurance
• One year of Occupational Therapy experience

Preferred:

• Experience in skilled nursing, inpatient rehabilitation, hospital, home health, or geriatric care
• Experience working with interdisciplinary teams
• An interest in preventative, community-based care
