The position requires California licensure as a Physical Therapist, CPR/First Aid, a valid driver’s license, and the ability to travel to multiple sites.

Qualifications

• Bachelor’s degree in Physical Therapy from a program approved by the COMA Education Commission (CAPTE) or equivalent.
• Current California Physical Therapy license.
• CPR and First Aid certifications on file.
• Valid California driver's license; reliable vehicle; ability to travel to multiple sites.

Responsibilities

• Collaborates with the primary care and interdisciplinary teams to evaluate participants and develop a plan of care.
• Instructs participants and caregivers in mobility-related devices and safe handling.
• Coordinates with vendors to obtain rehabilitative equipment and supplies.
• Takes part in the plan of care, including assessments, reassessments, and documentation.
• Treats participants to relieve pain, improve function, and maximize performance using therapeutic modalities.
• Maintains accurate documentation and participates in team conferences.
• Ensures safety, privacy, and professional conduct in all interactions.
