Job summary

Choice in Aging is seeking a qualified Physical Therapist to join our team serving frail elders and adults with disabilities in Pleasant Hill and Antioch.

Under the supervision of the Program Director, you will assess participants, develop IPCs, coordinate with the IDT, and deliver treatment plans aligned with Title 22 and California regulations. The ideal candidate has a PT license, 1+ year in rehab/home health, English proficiency, CPR, and reliable transportation.

Qualifications

• Current Physical Therapy license and at least one year's experience in a rehabilitation or home health setting within the past five years.
• Must be able to demonstrate the knowledge and skills necessary to provide care primarily to the frail and/or elderly populations including people with Alzheimer’s or dementia.
• Must communicate in English both verbally and in writing, second language a plus.
• Microsoft Office suite and health medial charting system experience is required.
• Knowledge of Title 22 regulations helpful.
• CPR Certification required.
• Must have reliable personal transportation, California Driver’s License, automobile insurance and clean DMV record.
• Fingerprint clearance required.
• Must travel to both Adult Day Health Care locations, Antioch and Pleasant Hill.

Responsibilities

• Provide an assessment and create a Physical Therapy IPC for every participant; all assessment paperwork must be completed by the first date of enrollment.
• Provide reassessment and quarterly reviews of the plan and function as a member of the Interdisciplinary Team (IDT).
• Ensure IPC is completed in accordance with program regulations and on a monthly basis.
• Establish treatment plans based on the assessment and schedule and perform treatment.
• Update and modify treatment plans based on subsequent reevaluations or with any changes of status or condition.
• Select evaluation and treatment methods appropriate to the participant’s physical and emotional status, diagnosis, and age group to formulate an appropriate care plan.
• Provide muscle, nerve, joint and functional ability tests; treatment to relieve pain, develop or restore function; assistance to achieve and maintain maximum performance.
• Educate participants and staff about physical therapy, injury prevention, ergonomics and ways to promote health.
• Complete new employee transfer training and provide human resources with training documentation for each newly hired employee.
• Develop activities for the general maintenance programs. Review and make changes as needed.
