Job summary

Center for Elders’ Independence is seeking a Physical Therapist to deliver restorative and rehabilitative services within an interdisciplinary care plan for elderly participants. The role emphasizes collaboration with primary care teams and family education to maximize independence and safety in daily activities.

The position requires CA licensure, a PT degree, CPR/First Aid, a valid CA driver’s license, and the ability to travel between sites and participants’ homes.

Qualifications

• Bachelor’s degree from a PT program approved by the Commission on Accreditation in Physical Therapy Education.
• Current license as a Physical Therapist by the Physical Therapy Board of California.
• Current CPR and First Aid certification on file.
• A valid California driver’s license and access to a reliable vehicle.
• Ability to travel to multiple work sites and participants’ residences.

Responsibilities

• Collaborates with the primary care and interdisciplinary team to evaluate participants and develop plans of care.
• Instructs participants and caregivers in use of mobility-related devices (wheelchairs, braces, crutches, canes, prosthetic/orthotic devices).
• Works with vendors to obtain rehabilitative equipment and supplies.
• Instructs team and family on the participant’s physical therapy program.
• Treats participants to relieve pain, restore function, and maintain maximum performance.
• Conducts initial assessments and completes reassessments per policy.
