Job summary

InnovAge Holding Corp. is seeking a Physical Therapist to evaluate and plan care for participants aged 55 and older, with acute restorative or maintenance needs.

You will integrate PT plans into the Plan of Care (POC) per physician orders, aligning with InnovAge policies and state regulations. You will collaborate with nursing and other therapy services, document care in charts, and educate participants and caregivers on DME and mobility strategies.

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• APTA-accredited program or CMS-approved equivalent.
• Active PT license in practicing state.
• At least 1 year experience with frail/elderly populations.

Responsibilities

• Screen and evaluate rehabilitation needs for acute, restorative, or maintenance levels.
• Collaborate with nursing staff regarding rehabilitation needs and refer to OT/ST as needed.
• Document changes in the Plan of Care and communicate with family and caregivers.
• Provide PT treatment, including modalities, exercises, and functional mobility training.
• Instruct participants and caregivers on DME use and loan policies.

Education

Bachelor’s Degree in Physical Therapy

APTA-accredited Physical Therapy program

Active Physical Therapist license

Minimum 1 year experience with frail or elderly
