Job summary

InnovAge is seeking a Physical Therapist to provide comprehensive PT evaluation and treatment for program participants aged 55+. The role integrates PT plans into the Plan of Care under physician orders, collaborating with nursing and rehab teams to optimize mobility and safety. A sign-on bonus of 5,000 dollars is offered.

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• PT program accredited by APTA or CMS-approved equivalent.
• Active PT license in practicing state.
• At least 1 year experience with frail or elderly clients.

Responsibilities

• Screen and evaluate participants for rehab needs across acute, restorative, and maintenance levels.
• Integrate PT plans into the Plan of Care per physician orders.
• Collaborate with nursing home staff on rehab needs.
• Refer to OT and ST with team approval as needed.
• Document plan of care changes and clinical notes in participant chart.
• Provide PT treatment including modalities, exercises, mobility training.
• Educate participants and caregivers on DME use and safety.
• Oversee and delegate PT plans to assistants per scope of practice.
• Participate as a member of the interdisciplinary team.
