Job summary

InnovAge Holding Corp. is seeking a Physical Therapist to deliver clinical PT evaluations and treatment plans for program participants aged 55 and older.

The role includes integrating therapy plans into the Plan of Care as directed by physician orders, and ensuring compliance with InnovAge policies and state regulations. Key duties include screening participants, coordinating with nursing staff, and providing patient education and documentation of DME and treatment.

Qualifications

• Bachelor’s Degree in Physical Therapy required.
• Program accreditation as per APTA/ CMS guidelines.
• Active Physical Therapist license in practicing state.
• Minimum of one year experience with frail or elderly individuals.

Responsibilities

• Screens and evaluates participants for rehabilitation needs and follows APTA ethics in daily practice.
• Collaborates with nursing home staff on rehabilitation needs and referrals to OT and ST.
• Documents changes in the plan of care and communicates with staff and family.

Education

Bachelor’s Degree in Physical Therapy

PT program accreditation

Active Physical Therapist license
