Job summary

Therapy Management Corporation invites a licensed Physical Therapist (PT) to empower adult/geriatric patients with compassionate, patient-centered therapy. You will deliver PT services aimed at improving independence, mobility, safety, and overall quality of life in a collaborative clinical environment.

You’ll partner with interdisciplinary teams, maintain thorough documentation, and adapt plans to support long-term rehabilitation goals while enjoying flexible scheduling and clinical autonomy.

Qualifications

• Licensed PT or eligible in the state of practice.
• Graduate of an accredited Physical Therapist Program.
• Interest or experience in skilled nursing, long‑term care or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments to improve mobility, strength, balance and independence.
• Develop and implement patient‑centered treatment plans for long‑term rehabilitation goals.
• Deliver hands‑on therapy interventions to support recovery and enhance quality of life in older adults.
• Help patients restore functional mobility and perform daily activities with confidence and independence.
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt plans as needed.
