Job summary

Qualifications

• Licensed Physical Therapist required.
• At least 1 year working with frail/elderly or approved training.
• Current CPR Certification required.

Responsibilities

• Evaluate and treat participants' mobility and motor assets and liabilities for functional performance.
• Provide goal-directed, activity-oriented treatment to promote independence in self-care and daily tasks.
• Educate participant, family or support system to ensure continuation of performance.
• Provide adaptive methods and equipment as appropriate to enhance performance.
• Participate in care plan meetings, IDT and relevant committees as determined by Therapy Director.
