Job summary

Blue Ridge Care in Winchester, VA, is seeking a Physical Therapist to join our PACE program, helping seniors stay independent and mobile in centers, homes, and community settings. As part of an interdisciplinary team, you will evaluate strength and mobility, design individualized treatment plans, mentor therapists, and support fall prevention and safety for participants in the PACE model of care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate participants' strength, mobility, balance, and functional abilities.
• Develop individualized treatment plans that promote independence and safety.
• Provide therapy services in the PACE Center, participant homes, and community settings.
• Collaborate with the Interdisciplinary Team to create holistic care plans.
• Educate participants and caregivers on mobility, exercise, fall prevention, and adaptive equipment.
• Recommend and evaluate durable medical equipment, orthotics, prosthetics, and assistive devices.
• Supervise and mentor Physical Therapy Assistants and rehabilitation staff.
• Support quality improvement initiatives that enhance participant outcomes.
• Help seniors achieve their highest level of function and quality of life.
